Dr. Judy Huang gives an excellent overview of Chiari malformation in this presentation from a meeting in Baltimore at Johns Hopkins University.

Chiari is a particularly "weird" condition to diagnose and treat. Some patients have confusing symptoms and may be commonly misdiagnosed with issues like migraine, fibromyalgia, or even depression. These misdiagnoses often "cover up" the real diagnosis of Chiari and can even mean that it takes an average time from first visit to proper diagnosis of at least 3.4 years!

Combine the difficulty of diagnosis with the complicated reality that not all patients do well with surgery? That makes Chiari malformation one of the most difficult to treat.
