Most surgeons associate dense cataracts with increasing difficulty during cataract surgery because they require more ultrasonic energy to break up the nucleus. However, a soft cataract with very little nuclear sclerosis can also be quite difficult. The nucleus tends to crumble and it becomes difficult to split the nucleus or even aspirate it without inadvertently damaging the posterior capsule. This is just such a case where we face these challenges plus others -- since the patient had prior 8-cut RK radial keratotomy.
